I Hereby release Tasha's Magicingaz Hair

Salon, from all claims, cause of action or liability and any or all injuries that may occur as a result of the services performed and or the materials used in accompany with the application of products furnished to me or applied to my hair, skin and nails. I represent that I have no history or occurrences of adverse skin reaction. I hereby assume all risk of personal injuries which I may sustain as a result of such application, where caused by Tasha's Magicfingaz Hair Salon. I realize in executing this release that I am freely and voluntary holding Tasha's Magicingaz Hair Salon harmless from all possible or actual conduct, by either of act or omission by them or any of them, whether in formulation manufacture or application of products used.

I further represent that I am at least eighteen (18) years of age and legally qualified to sign this release form or I have the consent of my parent or legal guardian as evidence by his/her signature below.

I understand that Tasha's Magicingaz

Hair Salon practice safe and sanitation that is in compliance with the Louisiana State board of Cosmetology and the Louisiana board of Barber examiners Department of labor. I understand that Tasha's Magicfingaz Hair Salon is a business that is opened to the public with a high volume of customers. This increases my chance of exposure to viral illnesses such as colds, flu, Covid-19,, strep and/or airborne contagion. I understand that sharing the same common areas makes me susceptible. I choose to receive service and release Tasha's Magicingaz Hair Salon, from all claims, cause of action or liability that may occur as a result of the service rendered.
